You will only be allowed to enter the building for the tryout if you have a Copy of Your Birth Certificate, plus you must print out the form below and have you and your parents fill it out and bring it to us on the Tryout Date.
Be in proper Basketball workout clothing. This means Basketball Shorts and Basketball Sneakers.
Also have a white T-shirt with you.
Do not be late.
The Tryouts will be held at I.S. 141 Gym on the 4th Floor.
The Queens RimRockers or The Beacon at I.S. 141 will not be responsible for any athlete that does not have a Beacon / I.S. 141 form filled out by a parent / guardian.

If any athlete fills out the form on their own and illegally signs the form without the parent or guardian knowledge, Queens RimRockers Basketball, Inc. and The Beacon at I.S. 141 are not responsible for any injuries or lawsuits. All rights are waived off completely by the family.
